Patient's lower eyelid developed an exaggerated bulge only upon smiling or squinting due to an overactive and overdeveloped (hypertrophic) orbicularis muscle located in the region just below the eyelashes.

1. Roll of tissue only when patient smiling
2. Skin incised just below the lashes
3. Dissection of small strip of muscle
4. Orbicularis muscle strip removed
5. Tiny amount of excess skin outlined
6. Excess skin removed; incision sutured
