Double-Layer Fascia-Fat (DLFF) Grafting
for Advanced Upper Eyelid Hollowness

A new approach for the restoration of
severely deficient upper eyelid/orbital volume

Before



This young patient shows a very deep and highly arched hollowness of the right upper eyelid due to the loss of orbital fat following an orbital fracture years earlier. Such hollowness is sometimes called a superior sulcus defect.

Previous operations designed to help this problem have yielded poor results. The patient had visited several oculoplastic surgeons and been advised that nothing could be done.

By utilizing fat extremely rich in its fascial component, a stable one-piece graft is able to be implanted without creating lumpiness or upper lid immobility.

Using the technique shown below, two such grafts were layered, thus allowing for more added volume and a safe and natural-appearing enhancement.

Operation

1. The first fascia-fat graft has been harvested and prepared. An eyelid incision is made to expose the muscle.

2. The first graft is placed into the proper anatomic location for orbital fat.


3. A second one-piece fascia-fat graft is then placed above the first, after which the skin is closed.

Results


Before DLFF and Four Days After

Volume has been substantially increased. Note (1) a much longer distance between the crease and the bottom of the eyebrow and (2) a much diminished depth to the previous hollowness.
